    Job Summary and Qualifications

     

    As an Advanced Clinical Pharmacist Medication Reconciliation, you will be responsible for the safe, effective, evidence-based, and cost-effective selection and utilization of medications for all patients. You will serve as a patient advocate when planning and implementing medication therapies and collaborating within the multidisciplinary team.

    What you will do in this role:

    + You will work directly with the certified pharmacy technicians provide a streamlined medication reconciliation transition into the hospital.

    + You will ensure the medication history and reconciliation is completed according to established policies, procedures and protocols.

    + You will provide ongoing mentoring and training to ensure the competency of medication history and reconciliation staff.

    + You will ensure safe, appropriate, cost-effective medication therapies for patients, according to the established policies, procedures, and protocols.

    + As needed, you will provide recommendations and/or literature to prescribers supporting the use of formulary agent, as well as, education to clinicians and patients regarding medication management.

    What qualifications you will need:

    + B.S. or Pharm D. degree in Pharmacy is required.

    + Graduate of an accredited PGY-1 pharmacy residency program is preferred.

    + Graduate of an accredited PGY-2 pharmacy residency program is preferred.

    + 3+ years hospital pharmacy supervisory experience is preferred.

    + Maintains current pharmacist licensure is required.

    + National Provider Identifier (NPI) is required.
