    The Social Services Specialist responsibilities include:

    + Being a member of a collaborative practice model that includes patients, nurses, hospital care coordinators, physicians, and other practitioners, caregivers, and the community

    + The Social Service Specialist II provides therapeutic interventions through assessment, coordination, referral and interdisciplinary planning

    + They are knowledgeable about treatment, social and economic implications for individuals and families as it pertains to their age, stage of development and discharge needs

    + Possess a clear understanding of discharge and transition planning and linkage with community resources

    + Demonstrates skills in planning, organizing, and managing multiple functions and complex processes

    Position details:

    + **Location:** St. Joseph's Hospital North, Lutz

    + **Status:** Part-time

    + **Schedule:** 8:00 AM - 4:30 PM

    + **Weekend requirement:** Every Saturday and Sunday

    + **On Call:** No

    Certifications and Licensures:

    + Preferred ACM (Case Management) or

    + Preferred CCM (Case Manager)

    Education:

    + Required Bachelor's Degree in Social Work, Sociology, or Related Field

    Experience:

    + Required 6 months Internship or

    + Required 6 months Social Work in an acute care or hospital setting
