    BayCare Health Systems is currently seeking a **Discharge Planner Pool** to join our outstanding and compassionate team at Morton Plant **!**

     

    + **Facility:** Morton Plant

    + **Location:** Clearwater, FL

    + **Status:** Pool, Hourly

    + **Shift Hours:** 8:00 am-4:30 am

    + **Weekend Work:** 4 Weekend Days/ Month

    + **On Call:** No

    The **Discharge Planner Pool** responsibilities include:

    + Provides assessment, linkage, monitoring, planning and advocacy for adult and or children experiencing acute mental health and or substance abuse issues in an acute care inpatient setting.

    + Possesses a clear understanding of discharge and transition planning and linkage with community resources.

    + Oversees legal status issues related to the individuals involuntary or in accordance with the Baker Act regulations.

    + Provides support as active member of the treatment team.

    + Will perform other duties as assigned by supervisor.

     

    _Travel may be required._

    Required Certification:

    + BLS (Basic Life Support)

    Required Experience:

    + Behavioral Health - 1 year

    + Child and Adolescent - 2 years (SIPP Only)

    + Acute Care - Preferred

    Required Education:

    + Bachelors - Counseling/Health Education/Related Field/Social Work/Human Services/Psychology/Nursing

    + Master's in human services - Preferred
