    Part-time, Monday through Friday, 15 hours per week.

    Description of Duties:

    This is a grant funded position. -Assist clients to attain their goals by identifying appropriate methods of achieving full program and medication compliance, introduce the clients to STAR Health Center (SHC)as well as all services that may be beneficial to clients -Educate new and existing clients on basis HIV/HCV transmission, disease, and treatment -Document all services provided within 24 hours -Work closely with the team to develop a plan for providing telephone abd face-to- face support to assigned clients -Maintain professional relationships with clients and ensure clients confidentiality -Complete any administrative work related to the program -Provide client support and assistance through monitored work which includes health education and medical escort

    Qualifications:

    REQUIRED: Lived experience with target population, experience working with HIV positive, HCV positive, substance abuse use, high-risk populations. Working with families with children. Ability and willingness to attend required trainings. Willingness to share personal experince of living with HIV/AIDS. Provide emotional support and willing to work different shifts as needed. PREFERRED: Able to offer practical self-care strategies to clients. Excellent interpersonal and communication skills; reliable, able to advocate for self and others. Computer skills and familiarity with office software applications.
