Job Description
Patient Navigation & Care Coordination
+ Serve as the primary point of contact for ACHD patients, assisting with appointments, referrals, and follow-up care.
+ Coordinate multidisciplinary care including cardiology, cardiac surgery, maternal-fetal medicine, electrophysiology, social work, and other specialty services.
+ Facilitate smooth transitions from pediatric to adult congenital care.
+ Ensure continuity of care by tracking diagnostic testing, procedure scheduling, and post-visit follow-up.
+ Identify patient needs and barriers, collaborating with the team to develop individualized care plans.
Patient Education & Support
+ Provide condition-specific education and resources to patients and families to promote understanding and self-management.
+ Educate patients on long-term follow-up care, lifestyle recommendations, and the importance of routine ACHD surveillance.
+ Support patients emotionally by promoting coping strategies and connecting them to appropriate support services.
Program Support & Communication
+ Communicate consistently with physicians, advanced practice providers, nursing staff, and testing departments to ensure efficient patient flow.
+ Assist in data collection for quality improvement, registries, and accreditation requirements (AHA/ACC ACHD Program standards).
+ Participate in ACHD team meetings, case conferences, and program development initiatives.
+ Collaborate with community partners, referring providers, and regional hospitals to enhance access to ACHD care.
Documentation & Compliance
+ Maintain accurate documentation of patient interactions, care plans, and navigation activities within the electronic health record (EHR).
+ Ensure all navigation practices comply with institutional policies, accreditation standards, and regulatory guidelines
Skills & Competencies
+ Strong understanding of congenital heart disease and adult cardiac care principles.
+ Excellent communication, patient advocacy, and interpersonal skills.
+ Ability to work independently while functioning as part of a collaborative, multidisciplinary team.
+ Strong organizational skills with the ability to manage complex care pathways.
+ Proficiency with electronic health records and clinical documentation.
Experience
+ 2–4 years clinical experience in cardiology, congenital cardiac care, critical care, or a related specialty required.
+ Experience with ACHD or pediatric congenital heart disease strongly preferred.
+ Care coordination or patient navigation experience is a plus.
+ Bachelor’s Degree required or 5+ years experience in lieu of a degree.
