JOB SUMMARY
A great opportunity for a Bachelor's prepared or a LPN to work in Care Coordination.
The Case Worker responsibilities include conducting care navigation activities to include, but may not be limited to, assisting those who are seeking primary care services, assisting clients with application assistance for various health care programs, identifying community resources, providing information and referral services, providing advocacy and conducting all appropriate project recordkeeping. This position requires the ability to work with low-income populations, meet project goals and objectives, exercise good judgment, and coordinate a wide range of health and social service organizations. The Case Worker will assist in the collaborative efforts with but not limited to RN or LPN for care coordination and service navigation to “at risk” individuals. The Case Worker promotes patient compliance, coordinates health and social services, supports patient follow-up and follow through, and promotes treatment adherence.
This position requires full understanding and active participation in fulfilling the Mission of Blessing Corporate Services. ICARE standards must be demonstrated. Adherence to Code of Conduct and other policies is required.
JOB QUALIFICATIONS
Education/Training/Experience:
REQUIRED:
+ Bachelor's degree in social service-related field OR graduate of a LPN School of Nursing.
+ Adequate transportation is a must.
License/Certifications/Special Requirements:
+ Valid driver's license and reliable transportation.
+ Possesses a current Illinois LPN license and/or any other LPN state license that an employee is required to work.
Pay Status:
+ Non-Exempt Hourly
