As a Home Health RN Case Manager, you’ll lead care where it matters most — in patients’ homes. You’ll build trust, guide families, and coordinate services that truly change lives. No rushed hallways. No shift whiplash. Just real nursing.

What You’ll Do (Every Day Has Meaning)

• Manage your own caseload with autonomy and support

• Supervise LPN visits and guide quality care

• Perform routine visits, evaluations, and starts of care

• Educate patients and families on CHF, COPD, Diabetes, wound care, and medications

• Collaborate directly with physicians to drive care plans

• Work side-by-side with PT, OT, and Speech professionals

What You Bring

• Active Kansas or Compact RN license

• Strong assessment and care coordination skills

• Confidence, organization, and compassion

• Valid driver’s license
