    Job Title : Permanency Planner - Homefinder Department: Foster Care

     

    Position Summary: Work collaboratively with foster parents, biological families, referring agency and other community resources to ensure continuity of treatment planning toward established discharge goals.

    Department Description:

    Gateway Longview’s Foster Care Department works with foster families and youth with a driving goal to achieve permanency. Foster Care currently works with approximately 75-85 foster families with a mix of foster, adoptive and kinship homes. Gateway-Longview services children ages birth – 21 while treating each case through a unique lens.

    Compensation

     

    Bachelor's Degree: $21.50 - $23.59 per hour

     

    Master's Degree: $23.30 - $25.38 per hour

    Full-Time, Monday - Friday

     

    Some evenings required as needed to assist with family and/or program needs

    Position Specific Duties/Responsibilities:

    + Implement placement development, and guide a treatment plan consistent with permanency plan of record. Assist with developing a concurrent plan for each youth and family.

    + Establish and maintain regular contact and therapeutic intervention with all parties (children, birth families, foster/adoptive families) ensuring diligent efforts toward the identified service plan. Provide ongoing collaboration with referral sources and community providers (I.E. Family court, Department of Social Services, Schools, Counselors, etc.).

    + Provide after-hour support and crisis intervention when necessary.

    + Ensure accurate, timely, comprehensive recording and filing of all components of the case record. Maintain casework efforts in accordance with ASFA regulations.

    + Effectively screen prospective foster/adoptive parent candidates for appropriateness and conduct orientation sessions, pre-certification training and complete necessary home studies in a timely manner.

    + Ensure and coordinate on-going in- service training and enrichment programs.

    + Review and document re-certification studies for resource parents.

    + Actively participate in professional and community organizations which provide access to an ongoing knowledge of current trends and needs in the treatment of children and families.

    + Strengthen skills and integrate application through ongoing in-service training to adhere to the minimum yearly training requirements.

    + Participate in all training opportunities, group consultations, etc. to ensure implementation of Solution-Focused Trauma Informed Care Principles including empowerment, safety, choice and trustworthiness.

    + Other duties as assigned

     

    Knowledge, Skills, and Abilities

     

    + Must have excellent communication skills

    + Ability to multi-task and be detail oriented

    + Ability to work independently and be an enthusiastic team player

    + Ability to work non-traditional hours as needed for the purpose of emergency support to the children youth and families served

    + Possess assessment skills, ability to engage families, and establish and maintain relationships with social services and other service providers

    + Specialized training in trauma treatment preferred

    + Ability to work effectively with clients, staff, families, and community contacts

    Qualifications

    + Bachelor’s Degree with relevant experience in social work or other human services field

    + Valid New York State driver’s license, adequate auto insurance, and reliable transportation to travel locally on a regular basis is required
