    Job Summary:

    You will provide direct case management services to an adult mental health population. You will spend 50% of your time face-to-face with consumers in their homes and the community.

    What You Will Do:

    + Facilitate and coordinate psycho-educational services for consumers and their family members on a variety of topics relating to wellness, health promotion, prevention, skill-building, and recovery topics.

    + Offer on-site and outreach services (including transport of consumers)

    + Fulfill case management services, including coordinating or directly facilitating linkages to services, service coordination, and treatment enrollment assistance.

    + Ensure consumers have community support in place to address identified needs.

    + Attend routine supervision and team meetings to review cases and discuss program operations.

    + Participate in supportive counseling and crisis intervention.

    + Participate in the assessment and evaluation of consumers at intake/admissions, during ongoing recovery/service delivery, and at discharge.

    + Conduct case management intake assessments and formulate plans of care.

    + Perform reviews of progress and evaluate the effectiveness of goals.

    + Work with administrative staff to ensure that all services are documented based on billing and regulatory standards.

    What We Need to See:

    + Master's Degree in a Mental Health-related field OR

    + Bachelor's Degree in a Mental Health-related field, plus one (1) year post-BA experience in the Mental Health field

    + Valid driver's license insurable by agency's automobile insurance
