As a Community Health Worker, you’ll work one-on-one with high-risk patients to set meaningful health goals and guide them toward success.
What you’ll do: Empower patients to achieve their health goals, provide support through health system navigation and community resource connections, offer counseling to encourage positive behavior changes, and collaborate with a dedicated multidisciplinary care team. This is an independent, impactful role that requires strong decision-making, problem-solving skills, and a drive for excellence. Summary:
+ Community Health Workers (CHWs) work one-on-one with high-risk patients to set and achieve health goals. Responsibilities include the identification and setting of health goals and follow-up provision of social support, health system navigation, community resource linkage and behavior change counseling. This is an autonomous position which requires a consistent drive for excellence, ability to make appropriate and timely decisions with minimal supervision, and effective problem-solving skills. CHWs are responsible for thorough and accurate documentation and working collaboratively within a multidisciplinary patient care team. CHW training must be completed upon hire.
Responsibilities:
+ Manage a caseload of 50–100 patients annually, meeting established care benchmarks: Contact 100% of patients weeklyResolve at least 60% of patient goalsAchieve a 90% patient satisfaction rate (10/10 Net Promoter Score)Ensure 60% of hospitalized patients attend post-hospital primary care follow-up within two weeksSupport 55% of outpatients in achieving their chronic disease management goals
+ Conduct comprehensive needs assessments in hospitals and primary care clinics using motivational interviewing to help patients set and achieve health goals
+ Provide personalized support, including: Addressing social challenges such as homelessness, substance abuse, and food insecurity
+ Assisting with medical record organization, follow-up appointments, and prescription fulfillment
+ Coaching patients on maximizing doctor visits
+ Helping with applications for programs like Medical Assistance and SNAP
+ Maintain weekly patient contact through calls or home visits
+ Collaborate with clinical care teams to share patient updates, address emergencies, and gather medical information
+ Co-facilitate weekly patient support groups and contribute to building a community resource directory Philadelphia
+ Document all patient interactions accurately and adhere to Penn Medicine values, policies, and procedures
+ Perform other duties as assigned to support the department and health system
Education/Requirements:
+ High School Diploma - Required
+ Comfortable with Home visits - Required
+ Knowledge of Philadelphia zip codes- Required
Skills Required:
+ Ability to exercise good judgment with patients and others involved in the delivery of the IMPaCT intervention
+ Ability to carry out written and oral instructions
+ Excellent oral and written communication skills
+ Proficient in MS Word and computer data entry
