Summary:
+ The Clinical Pharmacist Specialist in Critical Care provides advanced pharmaceutical care to critically ill patients across multiple intensive care units (ICUs). This role focuses on optimizing medication therapy, improving patient outcomes, and supporting interdisciplinary teams through evidence-based practice, education, and leadership.
Schedule:
+ Monday – Friday:8:00 AM – 4:30 PM
+ Weekends & Holidays:Not Required
Key Responsibilities:
+ ICU Coverage:Rotate among the following units (Every 2 month rotation):
+ Heart & Vascular ICU
+ Trauma ICU
+ Medical ICU
+ Neurology ICU
+ Clinical & Operational Excellence:
+ Optimize treatment plans for critically ill patients
+ Reduce medication errors and analyze trends for quality improvement
+ Facilitate communication for patients transitioning out of ICU
+ Develop cost-avoidance strategies and monitor impact
+ Education & Leadership:
+ Serve as a subject matter expert for medical staff, the Pharmacy Department, and Critical Care teams
+ Provide education to pharmacy and ICU staff
+ Coordinate pharmacotherapy initiatives and educational opportunities for pharmacists and other healthcare professionals
+ Precept and mentor learners, including pharmacy students and PGY-1 and PGY-2 residents (Infectious Diseases, Critical Care, Emergency Medicine)
+ Represent Pharmacy on selected clinical, operational, and departmental committees
+ Policy & Strategy Development:
+ Assist in managing medication shortages through guidelines and restrictions
+ Contribute to clinical policy and protocol development to enhance patient care
+ Lead or contribute to quality improvement projects; recommend updates to policies and protocols to optimize pharmacy services
+ Additional Responsibilities:
+ Lead/Collaborate in shortage-related efforts/initiatives
+ Participate in off-hours on-call rotation to provide clinical pharmacy support as needed
+ Education & Experience Requirements:
+ Required:
+ Bachelor of Science in PharmacyORDoctor of Pharmacy (PharmD) from an accredited College of Pharmacy
+ PGY-1 Pharmacy Practice Residency
+ PGY-2 Pharmacy Practice Residency in Critical Care (Preferred) or Emergency Medicine
+ Board Certified Critical Care Pharmacist (BCCCP)
+ Active Pennsylvania Pharmacist License
