The Bariatric and Metabolic Institute at Cleveland Clinic is excited to begin recruitment for an exciting new opportunity. The department has created a one year Advanced Practice Provider (APP) Fellowship for the purpose of training an APP to become a competent, autonomous provider in the sub specialty of Obesity Medicine. This is the first Obesity Medicine Fellowships in the country. This Fellowship will foster growth in both clinical and research arenas and build an extensive knowledge base on care of patients with obesity (including medical and surgical management) through a multidisciplinary approach. Obesity Medicine APP Fellows will learn how to treat the complex disease of obesity and related comorbidities by behavioral, nutritional, pharmacological and surgical means. We anticipate that trainees who complete the fellowship program will be prepared to pursue clinical and academic careers focused in obesity medicine and to become leaders in this emerging discipline. CV/Cover letter (uploaded with application) 2. 2 Letters of recommendations (uploaded with application) ELIGIBILITY: All applicants must submit a complete application by the stated deadline and have: – Completed, at minimum, a master’s program with degree conferred. – National board certification. – Licensing in Ohio obtained before start date without any restrictions. – Ability to be credentialed by The Cleveland Clinic Medical Executive Committee and Board of Governors. The APP Fellowship will be based at The Cleveland Clinic Main Campus in Cleveland, Ohio with the ability to travel to our satellite facilities as educational opportunities arise. The fellowship will be twelve months long with planned start in August/September 2024. Dr. Scott Butsch and Jennifer Mackey, CNP will chair the recruitment and management of the program along with other members of the department. The APP Fellow will spend approximately seventy percent of their time on direct clinical care of patients. The first quarter of the fellowship will be spent on foundational education the science of weight regulation and clinical training in obesity medicine. Fellows will work with Board certified obesity medicine specialists, bariatric surgeons, advanced practice providers, dieticians, and psychologists to learn about the complex disease of obesity. Teaching methods will include modeling by staff physicians and APP’s, participation in weekly conferences, direct 1:1 instruction by staff physicians, shadowing collaborative departments, use of scientific literature and information technology, and recommended reading. Fellows will manage their own patient panel with physician and APP oversight. The fellow will be expected to contribute to ongoing research projects within the department and section and if the opportunity arises present at a sponsored meeting. The fellow will attend a national conference. Funding for conference attendance will be provided in similar fashion to employed APPs. The fellow will be required to complete a Capstone Project that will be due during the fourth quarter prior to the completion of their fellowship. The Capstone Project will be in an area that is of interest to the fellow and specific plans and requirements will be individualized. Examples of acceptable Capstone Projects include a State of the Art presentation, publication of a peer-reviewed article (original research or review article), or completion of a quality improvement project with presentation to the Obesity Medicine Section. Compensation and Benefits: The fellow will be hired in a full-time, salaried position not to exceed one full year of employment. The fellow will be eligible for the full-time benefits as described in the Total Rewards Brochure. The salary will be paid to the employee on a semi-monthly basis. If the fellow is successful in the role, upon completion of the termed program the fellow may have the option to apply for an open position within the department or within the Cleveland Clinic enterprise. If the fellow does not secure employment within the Cleveland Clinic, their employment will be terminated per the terms of this fellowship. Employment at the end of the fellowship is not guaranteed. APRN/PA learns the role of an Advanced Practice provider under supervision of the APRN/PA preceptor and/or Physician preceptor for no longer than 12 months.

Responsibilities:

  • Conducts thorough medical histories, performs complete physical examinations (where indicated), initiates appropriate lab, radiology tests or other special tests required for evaluation of illness, and scrutinizes lab data on new and former clinic patients and hospital admissions to establish a record of the patient's current health status and to develop a working diagnosis and treatment plan with direct review by a physician.
  • Works under direction of supervising physician within the confines of state law
  • Performs and interprets, at least to the point of recognizing deviations from normal, common laboratory, radiologic, cardiographic and other routine diagnostic procedures used to identify pathophysiologic processes as privileged by CCF Credentialing Board.
  • Performs routine procedures such as injections, immunizations, suturing and wound care, managing simple conditions produced by infection or trauma, assisting in the management of more complex illness and injury which may include assisting surgeons in conducting operations and taking initiative in performing evaluation and therapeutic procedures in response to life-threatening situations, as privileged by CCF Credentialing Board.
  • Performs and interprets, at least to the point of recognizing deviations from normal, common laboratory, radiologic, cardiographic and other routine diagnostic procedures used to identify pathophysiologic processes as privileged by CCF Credentialing Board. Performs specialized procedures, including bone marrow aspirations, biopsies and harvest, blood draws, designated IV starts, spinal taps, thoracentesis and paracentesis as well as other specific procedures as directed by the physician, following approval by CCF Credentialing Board.
  • Provides information and answers routine questions of patients regarding their disease, including preventive health maintenance topics such as diet, weight, smoking and topics related to medical specialty, with basic recommendations.
  • Educates patients regarding their disease, treatments, related drug and treatment side effects and hazards.
  • Assists with record keeping, completes appropriate patient medical chart, and transcribes lab data and routine correspondence, as directed and reviewed by physician.
  • Refers patients to physicians as appropriate for consultation or for specialized health resources and treatment.
  • Ensures continuity of care by serving as a liaison between patient and other members of the multi-disciplinary care team or with other specialty areas as necessary.
  • Performs research follow-up studies as directed by the supervision of a physician. Assists in data analysis.
  • Participates in quality monitoring thru the review of records and treatment plans for patient outcomes on a periodic basis to assure quality care.
  • Facilitates appropriate length of stay, discharge planning and compliance with regulatory standards for inpatient management.
  • Other duties as assigned.

Education:

  • APRN: EDUCATION: MSN required upon hire or within 3 years of hire for external hires and internal hires moving from a non-APRN licensed role to an APRN licensed role.
  • Graduate of an accredited ARPN training program.
  • PA: EDUCATION: Graduate of an A.R.C.-approved Physician Assistant Program.
  • Master's degree in Physician Assistant Studies and/or applicable state license through the State Medical Board for certificate to practice

Certifications:

  • APRN: Currently licensed as a Registered Nurse and Advanced Practice Registered Nurse in applicable state.
  • National certification as Advanced Practice Registered Nurse by nationally accredited organization recognized by state Board of Nursing, current licensure by applicable state Board of Nursing, (including certificate to prescribe if applicable).
  • Must be privileged by CCHS’s credentialing board
  • Basic Life Support (BLS) through the American Heart Association (AHA) required.
  • Advanced Cardiac Life Support (ACLS) or Pediatric Advanced Life Support (PALS) through the American Heart Association (AHA); or Neonatal Resuscitation Program (NRP) through the American Academy of Pediatrics (AAP) may be required based upon work setting.
  • PA: Current certification as a Physician Assistant through the NCCPA (or successor).
  • LICENSED BY the State Medical Board OF OHIO AS A PA-C.
  • Must have a Physician Supervision Agreement and/or signed addendum to Physician Supervision Agreement.
  • Must be privileged by CCF's Credentialing Board.
  • If candidate has held a license to prescribe within another jurisdiction and needs to obtain a Master's degree in order to apply for CT prescribe from applicable state, the candidate will be subject for review for eligibility for hiring within our system.
  • Basic Life Support (BLS) through the American Heart Association (AHA) required.
  • Advanced Cardiac Life Support (ACLS) or Pediatric Advanced Life Support (PALS) through the American Heart Association (AHA) may be required based upon work setting.

Physical Requirements:

  • Typical physical demands include the ability to walk and stand for long periods of time.

  • Requires corrected vision and hearing to a normal range.
  • Occasionally lifts and carries items weighing up to 50 pounds.
  • May require exposure to communicable diseases and/or body fluids.
