Job Summary: The Medical Record Completion Specialist is responsible for ensuring the accuracy, completeness, and compliance of inpatient and outpatient medical records in accordance with Saint Francis Health Care System, State, Federal, and accreditation standards. This role supports healthcare providers and hospital staff by managing incoming requests, correspondence, medical record deficiencies, facilitating timely chart completion, and ensuring data integrity for coding and compliance purposes.
Minimum Education: High school diploma or GED.
Licensure, Registration and/or Certification: None.
Work Experience: Minimum 1 year in healthcare related field.
Knowledge, Skills and Abilities: Knowledge of Microsoft 365 and other applicable software. Excellent communication skills, both written and verbal that present clear and concise information. Ability to work independently and collaboratively in a fast-paced environment, managing multiple priorities with competing deadlines. Sound ability to maintain professional and courteous and demonstrate flexibility and adaptability in a dynamic work environment. Ability to input a minimum of 6,000 keystrokes per hour with a 6% or less error rate on the Data Entry Test.
Essential Functions and Responsibilities: Conducts detailed analysis of inpatient and outpatient medical records and assigns medical record deficiencies to the appropriate provider for completion according to regulatory State, Federal, and The Joint Commission. Facilitates and monitors the medical record completion process by assisting physicians and hospital staff with timely documentation and collaborate with physicians, departments, and clinical teams to obtain missing documentation required for coding and regulatory compliance. Audits provider completion activity and enforce Medical Staff Rules and Regulations related to record completion. Performs quality control reviews to verify the integrity, completeness, and legibility of medical documentation and utilize data analytics tools and facility reports to track deficiencies, monitor trends, and support compliance improvement efforts. Manages and monitor analysis-specific queues, validation queries, and workflow timelines, as well as research chart completion issues and update deficiencies to ensure accurate, compliant, and efficient record processing. Submits and track chart correction tickets to address identified issues with medical record completion or documentation discrepancies. Provides office and clerical support; receive incoming telephone calls and assist caller as needed; monitor incoming facsimile correspondence and route to appropriate department for processing; and performs other duties as required in all areas of health information.

Working Relationships: Works with internal and/or external customers via telephone or face to face interaction. Works with other healthcare professionals and staff.
