Practice within physician offices within Scotland Health Care System to provide protocol-driven, evidence-based patient care. Educates and consults with patients and providers regarding use of medications, disease management, potential drug interactions, and monitoring of drug therapies. Areas of focus may include, but are not limited to metabolic disease, cardiovascular disease, and anticoagulation. Team based care including working with multidisciplinary teams and care management may be involved.
Responds to requests for drug information from health care providers and patients. Evaluates the usefulness of biomedical literature gathered and formulates responses to drug information requests based on analysis of the literature. Provides medication-use education and counsels patients and caregivers. As CPP, performs co-visits with patients and prescribes medications as appropriate.
