Position Summary: The Children & Families Support Initiative (CFSI) team brings community-based services to youth aged 10+ and their families. With a focus on collaboration, the CFSI team maintains manageable caseloads and works closely with schools, courts, and community partners to provide strength-based guidance and informed decision-making. The Case Manager plays a pivotal role in empowering youth and families by assessing their needs and crafting personalized success plans alongside the CFSI Director and Youth Coaches. This position involves connecting families to essential community resources like mental health services, substance abuse treatment, education, and employment opportunities. The Case Manager ensures seamless coordination through youth-centered team meetings, timely documentation, and collaboration with DSS, Probation, and the Court. They also represent the agency in Family Court, County Advisory Boards, and Coalitions, championing the voices of the youth they serve. With a deep sensitivity to trauma and unique service needs, this role is at the heart of driving impactful change and fostering brighter futures.
Responsibilities:
+ Assess the needs of youth and families in collaboration with the CFSI Director and Youth Coaches.
+ Develop and implement individualized success plans, linking families to community-based services such as mental health support, substance abuse treatment, educational opportunities, and employment resources.
+ Maintain timely and accurate documentation, including client records, progress notes, and case-specific materials.
+ Coordinate youth-centered team meetings and communicate with DSS, Probation, and the Court.
+ Attend Family Court, Probation, and DSS meetings, and participate in County Advisory Boards and Coalitions as requested.
+ Serve as a key member of the youth's interdisciplinary team, demonstrating sensitivity to trauma history and unique service needs.
Requirements
Education
+ Associate's in human services or related field required
Experience
+ 2 years in child welfare, juvenile justice, or community engagement services required
+ Driver's License and ability to maintain insurability throughout employment required
