    Job Description

    Homeward Bound is seeking a dependable and compassionate Appointment Float DSP to provide high-quality care to individuals with developmental and physical disabilities. In this role, you’ll travel between sites to support medical appointments and train new team members—making a meaningful difference every day.

    Key Responsibilities:

    + Transport and accompany individuals to routine and follow-up medical appointments

    + Communicate between medical providers, nurses, and direct care teams

    + Deliver respectful, person-centered care in home and community settings

    + Mentor and train staff across multiple HBI locations

    + Support daily living needs such as hygiene, mobility, and meals

    + Maintain accurate documentation on health status and program goals

    Requirements

    + 2 years of direct care experience, preferably with individuals with disabilities

    + Valid MN driver’s license and clean driving record

    + Ability to lift 60 lbs and meet physical demands of the role

    + Must pass a MN Department of Human Services background study

    + CPR/First Aid certification (or willingness to obtain)

    + Reliable, organized, and flexible to adapt to schedule/location changes
