    Position Overview:

    The responsibility of the Transition and Discharge Coordinator is to supplement and assist the Diagnostic clinical team in addressing the needs of the children and families to prepare for their discharge. Under close supervision one may be:

     

    + Processing intake and discharge tracking forms as well as follow up assessment forms

    + Formulating and implementing appropriate treatment and discharge plans for children and families.

    + Coordinating with the social workers and other professionals as to carefully planning for their discharge of each client and family to provide a linkage in the local communities to ensure a successful discharge from the Diagnostic and reintegration in the community or placement.

    + Collaborating with members of the treatment team, as well as professionals from other community-based and foster care agencies to ensure the most effective treatment and discharge plan for each child.

    + Documenting all interventions and required reports that have to do with discharge in a timely and professional manner.

    + Organizing and tracking of client services

    Position Qualifications:

    BSW or BA in human services field and two years experience in providing direct services to or linking children with serious emotional disturbance to a broad array of services may be substituted with coursework towards a degree in Counseling, Social work or human services field. Experience may be pre or post degree. Ability to function with expanded autonomy based on the knowledge of case worker/coordinator functions. Must have strong interpersonal skills and documentation skills. Must be cleared by the State Child Abuse Register. Driver’s license. Proficient in the use of computer.
