    Position Details

    + Schedule: Monday–Friday, 8:00 a.m. – 3:30 p.m. with ½ hour unpaid lunch

    + Rate of Pay: $21.51 per hour

    + Status: Internal posting only

    Key Responsibilities

    + Assist individuals and families referred to Coarc through the OPWDD Front Door process with all activities related to OPWDD eligibility and Medicaid enrollment.

    + Monitor each step of the Front Door process and collaborate with OPWDD Front Door Liaisons to ensure timely and accurate submission of required documentation.

    + Track documentation submissions and complete quarterly contract reporting in the format required by OPWDD and the Supervisor.

    + Review consent forms, individual rights notifications, and other required documentation to ensure accuracy and compliance.

    + Complete all required regulatory and agency service documentation within established deadlines.

    + Perform additional duties as assigned by the Supervisor.

     

    Education & Experience Requirements

     

    + Associate's degree in a health or human services field OR valid license as a Registered Nurse; and

    + One (1) year of experience working with individuals with developmental disabilities OR one (1) year of experience as a service coordinator with any population.

     

    Certificates / Licenses

     

    + Valid NYS Driver's License required.

    + OCFS Child Abuse Registry Clearance required.
