    Primary Responsibilities:

    + Successfully performs all the duties of a Care Manager for assigned caseload see the Care Manager Position Description

    + Management of Care Managers

    + Provides supervision of service delivery and record keeping (per regulations and LIFEPlan’s policies, procedures, and standards) with a focus on meeting the needs of the members and families served.

    + Assesses, and recommends qualified staff for hire.

    + Provides training and feedback to Care Managers as deemed appropriate based on feedback from members and families, quality metric reporting, and/or chart audits completed by the Care Management team, Quality Assurance team, or Compliance team.

    + Ensures that caseload files and electronic records are current and that all documentation and billing standards are met, including progress notes.

    + Serves as a liaison whenever appropriate with families, outside agencies in order to address concerns and/or further develop or enhance LIFEPlan services.

    + Manages the performance of all assigned staff by continuous and regular supervision, coaching, and performance evaluation.

    + Completes annual performance evaluations.

    + Uses bi-weekly meetings to address performance concerns timely and as appropriate; ensures issues do not go unresolved (works with his or her Director, Regional Vice President and the Vice President of Human Resources as needed)

    + Maintains appropriate documentation involving supervisory interactions with staff.

    + Monitors and approves time and attendance of direct reports and ensures compliance with applicable labor laws and LIFEPlan policies.

    + Ensures all required trainings and meetings are completed and documented according to OPWDD regulations, along with LIFEPlan’s policies and procedures.

    + Attends in-service training courses to continue to develop and strengthen management and supervisory skills.

    + Participates in, and facilitates staff meetings, and any other meetings relevant to departmental needs.

    + Assumes On-Call responsibilities on a rotational basis and as needed.

    + Performs all other duties as designated by direct supervisor and the Regional Vice President for Care Management.

    + Discusses and reviews issues with individuals in multiple disciplines to arrive at the best possible outcome. Knowledge of when to report incidents to CPS, APS, and/or law enforcement.

    + Effectively communicates incidents to the Regional VP and Compliance in a timely manner.

    + Assists Care Managers through the reporting process and supports staff to execute plans ensuring health and safety.

    Key Duties & Responsibilities for all Leadership/Management Positions:

    + Ensures compliance with all pertinent government regulations, and LIFEPlan CCO’s policies, procedures, and operating standards.

    + Implements a continuous quality improvement process for department services and administrative functions.

    + Assists in the development, implementation and administration of Human Resources policies and programs including compliance with Equal Employment Opportunities and Affirmative Action Programs.

    As a member of the LIFEPlan Management Team, the Care Manager Supervisor sets an example and models the expected performance for each Care Manager. The Care Manager Supervisor applies his/her expertise and experience to support and ensure the Care Manager’s success. The Care Manager Supervisor may support members directly as needed.

    Qualifications

    Qualifications:

    + Bachelor’s degree is required.

    + Master’s degree with a minimum of one year of experience plus minimum experience listed below.

    + Specialized training, license, certification, skills in:

    + Knowledge of community resources, state agency resources including OPWDD services and regulations, Electronic Health Systems, state and federal regulations, SSI/Medicaid eligibility, other public benefits and entitlements and funding sources

    + A minimum of 3 years of job-related experience, ideally working with people with disabilities; case management, Waiver services, substance abuse, social work and other relevant experience will also be included.

    + Experience as a team leader/supervisor is highly preferred.

    + Language/Communication Skills. Communicates to providers, both internal and external, regarding the needs of the people receiving services, goals, and areas of concern such as gaps between desired and available services. The ability to communicate clearly and effectively, verbally, in writing, and through the use electronic communication, is essential. The ability to negotiate and resolve conflicts with others, understand another individual’s point of view and carry out complex written, and verbal instructions is of importance. Exceptional listening skills. Ability to develop and maintain ongoing and effective relationships with internal and external stakeholders.

    + Computer Skills. Basic computer skills (windows, email/Outlook, Adobe Acrobat, Word, Excel, print, scan, download information) are required. Ability to learn and train others on web-based platforms and portals.

    + Math Skills: Ability to perform math functions and use formulas to analyze data and prepare reports.

    + Reasoning Ability: Must have the ability to apply relevant state/federal regulations to a variety of circumstances - e.g., NYS Department of Health Home, OPWDD Regulations: 624, 633, and 635 and the Justice Center regulation 625.

    + Knowledge Required: Extensive knowledge regarding community and state agency resources, state/federal regulations, I/DD, SSI/Medicaid eligibility, funding sources, benefits and entitlements, along with health and safety needs, etc.

    + Approach to Work: Demonstrates respectfulness, professionalism, and effective teamwork, building rapport, building constructive and effective relationships with community members, using diplomacy and tact, maintaining composure under pressure, relating well to all kinds of people (inside and outside the organization).

    + Working Environment: Majority of hours worked will be Monday Friday, between the hours of 8a 6p; expected to be flexible with work hours in order to accommodate the individuals and /or families schedules. Certain deadlines and unanticipated developments may require work during evenings and on weekends, as well as hours that exceed the standard number of hours. May be working in a variety of settings and neighborhoods, including being in people’s homes that are not optimally safe and /or ill maintained. Travel is required. Must have reliable transportation with valid vehicle insurance, a valid NYS Driver’s License, with an acceptable driving record; and may have to drive for extended periods of time, and in unfavorable conditions. Must have the ability to act/think quickly under stressful situations.

    + Physical Demands: Extensive travel (estimated at 50%) is required within assigned regions and in order to meet the requirements of the position for face-to-face meetings with Individuals served, to attend meetings/trainings, so ability to drive (with own vehicle), sit for long periods, overnight stays as needed. Occasional lifting of up to 10-15 lbs. Possibility to assist with a transfer or pushing of a wheelchair.

    + Problem Solving: Ability to function with a high degree of autonomy requiring occasional reliance on their direct Manager, Director or Regional VP. Often presented with complex issues or concerns that require problem solving related to general linkage and referral, financial issues, educational concerns, or advocacy.

    + Decision Making: Independent and timely decisions. Exceptional judgement on when to escalate to the Regional VP, Compliance, etc. Must be able to work independently with members and their family to review issues. When guidance or supervision is required outside of business hours, support is provided through LIFEPlan’s on-call system.

    + Consequence of Errors: Judgement is critical. This position is ultimately responsible for the quality of care and services to the individuals within assigned area. Non-compliance with regulations and LIFEPlan policies and procedures may cause harm to the individuals we support, as well as have financial/legal consequences for LIFEPlan and interfere with the LIFEPlan’s positive reputation as a provider of quality services to the community.

    + Confidential Information Handled: High level of confidentiality and discretion is required at all times. Frequently handle Protected Health Information and must abide by HIPAA privacy and security regulations, as well keep other sensitive information confidential
