Amergis is seeking a qualified Case Manager to work at a leading provider of home and community-based specialized health care. This would be a full-time day-shift role and would be an immediate start. See below for full details:

Schedule: Monday-Friday 8am-5pm

Pay Rate: $28/hr

We will reimburse for mileage*

13 Week assignment with an option to extend or go internal!

Experience Required:

+ QIDP certification REQUIRED

+ Must have experience with IDD Services

+ Bachelor's degree in human services or equivalent in education and exp required

+ Valid driver's license, registration, and insurance

+ Excellent Documentation and Communication skills preferred

Caseload: 4-6 home visits a shift, 3 individuals in each home, all in same general area
