Position Summary:
The Home Health Nurse provides high-quality, compassionate nursing care to patients in their homes. This role involves clinical assessment, care coordination, patient education, and documentation using the RXNT Electronic Health Record (EHR) system. The nurse is also responsible for accurate time tracking by clocking in and out of patient visits using Sandata.
Key Responsibilities:
Clinical Care
• Perform skilled nursing visits in the patient’s home, following the Plan of Care and regulatory guidelines.
• Conduct comprehensive assessments, including vitals, wound evaluations, medication reviews, and
patient/caregiver teaching.
• Provide direct patient care such as wound management, catheter care, injection administration, chronic
disease management, and other skilled nursing procedures.
Documentation & Compliance
• Document all patient interactions, assessments, interventions, and care coordination in RXNT in accordance
with agency standards and state/federal regulations.
• Ensure documentation is timely, accurate, and supports all billable services.
• Use Sandata for electronic visit verification (EVV), clocking in and out of each patient visit.
• Comply with HIPAA, OSHA, and all other regulatory and agency policies.
Care Coordination
• Report changes in patient condition to the clinical supervisor and interdisciplinary team promptly.
• Collaborate with physicians, therapists, home health aides, and other care team members to ensure
coordinated and effective patient care.
• Participate in case conferences and staff meetings as required.
Patient & Family Education
• Educate patients and caregivers on disease processes, medications, wound care, safety, and self-
management strategies.
• Support patient empowerment by reinforcing care plans, treatment goals, and proper use of medical
equipment.
Qualifications:
• Required: Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of practice.
• Minimum of 1 year of nursing experience; home health experience preferred.
• Familiarity with RXNT or other EHR systems preferred; training available.
• Experience using Sandata or other EVV systems preferred.
• Strong clinical assessment, problem-solving, and communication skills.
• Ability to work independently, manage time effectively, and travel to patient homes.
• Current CPR certification, valid driver’s license, and reliable transportation.
Physical & Work Requirements
• Ability to lift, push, or pull up to 25 lbs.
• Ability to stand, walk, and navigate various home environments.
• Travel within assigned geographic area.
