Are you a **Certified Nursing Assistant (CNA)** with Skilled Nursing Facility (SNF) experience who thrives on building relationships and making a difference in the lives of seniors?
At Gentiva Hospice, we believe every day is an opportunity to bring comfort, dignity, and peace to patients and families. We’re currently seeking a compassionate, dependable CNA to serve patients in **Skilled Nursing Facilities throughout the Dallas area** —while also serving as a **liaison between Gentiva and facility staff** to ensure quality, coordinated care.
This unique role is ideal for a CNA who enjoys hands-on care **and** the opportunity to collaborate with facility teams, advocate for patients, and represent hospice services in a professional, positive way.
What You’ll Do:
+ Provide personal care and support to patients residing in SNFs
+ Assist with daily activities including hygiene, grooming, mobility, and comfort needs
+ Observe and report changes in patients’ conditions to the care team
+ Build strong, trusting relationships with facility staff, residents, and families
+ Support facilities in coordinating hospice services and care plans
+ Represent Gentiva Hospice at facility meetings and help educate staff about hospice philosophy
+ Maintain timely, accurate documentation and uphold safety standards
About You
Qualifications – What You’ll Bring:
+ Active CNA license in the state of employment (Required)
+ Current CPR certification (Required)
+ Driver’s license and reliable vehicle with insurance (Required)
+ Six (6) months or more of direct patient care experience (Preferred)
+ Comfortable working in home settings, hospice facilities, or long-term care environments
+ Demonstrated compassion, patience, and ability to handle emotional situations
+ Willingness to work independently and adapt to changing patient needs
Preferred Background (Not Required):
+ Experience in home health, hospice, palliative care, or oncology
+ Prior roles in hospital nursing, ICU, geriatrics, med-surg, ER, telemetry, or nursing homes
+ Exposure to end-of-life care, wound care, or admissions support
