**Pay Range:** $27.47 - $43.27 per hour

Summary:
The Social Worker LCSW provides mental health and psychosocial support to patients seeking gender affirmation surgeries. The Social Worker connects patients to relevant resources both internal to Rush and in the community and supports relevant programming for Affirm patients. This role collaborates with the Patient Care Navigators to engage patients through pre-surgical assessments, the provision of surgical letters of readiness, discharge planning, post-operative follow-up, and individual psychotherapy. This position will also have the opportunity to receive individual and group supervision while having access to continuing education opportunities throughout the hospital system. Responsibilities:

• Collaborates with Patient Care Navigators to perform pre-surgical assessments of psychosocial and psychosocial needs and provide ongoing support as needed.
• Participates in pre-admission patient planning calls and collaborates with inpatient case managers to develop and execute discharge plans for surgical patients.
• Maintains relationships with referring Rush clinics through consistent and timely communication.
• Connects patients and their families to relevant resources, both at Rush and in the community, to address their needs.
• Provides psychiatric evaluations and produce letters of readiness for patients seeking gender affirmation surgery.
• Completes all documentation and billing of patient sessions and other patient interactions in Epic software.
• Maintains relationship with Affirm providers through consistent and timely communication and participation in interdisciplinary meetings as scheduled.
Other information:
Required Job Qualifications:
• Master’s degree in Social Work required.
• Current State of Illinois Licensed Clinical Social Worker (LCSW) licensure required.
• At least three years of experience in healthcare, care management, mental health, and/or social services required.
• Clinical experience with relevant population(s) required.
• Proficiency with the Microsoft Office Suite.
• Strong active listening, critical thinking, judgment and decision-making skills.
• Ability to approach clients and workplace from a trauma-informed and health equity lens.
Preferred Job Qualifications:
• Experience with evidenced-based interventions preferred.
• Experience with high risk and/or complex populations preferred.
