    Job Summary:

    This pharmacist will primarily provide care and oversight for the safe and efficient transitions of care among patients admitted and discharged from the hospital. They will work as part of a dedicated team of transitions of care pharmacists, in collaboration with other healthcare professionals. This position will collaborate closely with the health system’s outpatient pharmacy to help execute our meds to beds program. This includes coordination of medication orders, medication delivery, and patient counseling. In addition to transitions of care activities, this position will also provide inpatient pharmacy services, particularly when scheduled on weekends and potential evening shifts.

     

    This pharmacist will practice within a progressive clinical pharmacy model at Upstate University Hospital. Applicants will join a talented team of pharmacy personnel to deliver exceptional pharmaceutical care. Pharmacists at Upstate participate in all aspects of care, including clinical patient monitoring, sterile product preparation/oversight, medication distribution, transitions of care management, and patient counseling/education. As an academic medical center, opportunities for teaching and scholarship exist and are encouraged. Pharmacists at Upstate have the opportunity to practice at the top of their pharmacy licensure.

     

    Job duties for the position include but are not limited to: Interpretation of medication orders and prescriptions to ensure they are complete, safe, rational, and compatible with institutional policy. Pharmacists have responsibility for preparing compounds and dispensing medications for inpatient and ambulatory care use. This includes the preparation of injectable sterile products and non-sterile compounds. Pharmacists may be responsible for performing duties in various practice locations, including inpatient, emergency departments, ambulatory care clinics, and transitions of care settings. Pharmacists at Upstate are expected to provide information to health care practitioners to promote the safe and rational use of medications. Furthermore, pharmacists shall provide drug monitoring services, maintain pharmacy records according to law and institutional policy, interprets policy and procedures for hospital staff, participate in teaching programs for the department and hospital staff, and participate in the department’s quality assurance programs.

    Previous hospital experience preferred. Previous experience with hospital-based transitions of care activities preferred.
