    Job Summary and Qualifications

     

    The Sepsis Coordinator assists with the coordination of the Sepsis Program to ensure high quality, patient-centered care by supporting the daily operations of the management of sepsis patients. This person works in collaboration with the medical and hospital staff to provide multidisciplinary care to the septic patient.

     

    Provides clinical, professional expertise for patients, families and staff members. Scope of practice includes providing organized care to the patient with sepsis with focus on health promotion and disease prevention.

     

    Rounds on sepsis patients in the emergency department and inpatient units and coordinates care in critical care/sepsis patient care management, ensuring optimal patient care management and outcomes.

     

    Monitors sepsis bundle compliance and facilitates compliance with the bundle metrics during concurrent rounding. The Sepsis Coordinator also functions as an educator.

     

    Facilitates the quality improvement process and collects and analyzes data surrounding the sepsis patients to assist with documentation, coding, clinical care, and outcomes.

     

    Conducts research for best practices and refines/implements sepsis tools and practices to become the standard of care within the facility.

     

    Serves as chairman/co-chairman of the Sepsis Committee and maintains active involvement in nursing and hospital committees as necessary.

     

    Collaborates with all members of the interdisciplinary team to achieve productive movement of the best practices within the organization.

     

    Applies good verbal and nonverbal skills to achieve service goals of customer service and satisfaction

     

    The Sepsis Coordinator collaborates with the Quality Leadership Team and other stakeholders to monitor processes, identify opportunities, obtain analyses, provide constructive input and channeling concerns appropriately, in compliance with applicable laws, regulations, and other governmental requirements. The Sepsis Coordinator highlights priorities through data analysis and trending, incident response, and assists with staff education to advance patient safety and control and minimize risks at the facility.

    What qualifications you will need:

    + Bachelor’s Degree in Nursing or health-related field2+ years clinical experience in a healthcare setting1+ years experience in healthcare quality setting (Preferred)2+ years experience with computer programs, database programs, spreadsheets and project management
