Position: Intensive Case Manager

Key Responsibilities:

• Provide comprehensive case management services to individuals with complex needs.

• Coordinate with healthcare providers, social services, and community resources to ensure holistic care.

• Monitor and evaluate client progress, adjusting care plans as necessary.

Qualifications:

• Bachelor's degree in social work, psychology, or a related field required; Master's degree preferred.

• Experience in case management or a similar role.

• Strong organizational and communication skills.

• Ability to work independently and as part of a multidisciplinary team.
