    Status:** **Pool, Exempt: No

     

    Shift Hours:** **8:00am-4:30pm

     

    Weekend Work:** **Every Other

     

    On Call:** **No

    Discharge Planner Pool - Responsibilities:

    + Provide assessment, linkage, monitoring, planning and advocacy for adult and or children experiencing acute mental health and or substance abuse issues in an acute care inpatient setting.

    + Possesses a clear understanding of discharge and transition planning and linkage with community resources.

    + Oversees legal status issues related to the individuals involuntary or in accordance with the Baker Act regulations.

    + Provides support as active member of the treatment team.

    + Travel may be required.

    + Will perform other duties as assigned by supervisor.

    Experience:

    + **Required:** 1 year of Behavioral Health and 2 years of Child and Adolescent (SIPP Only)

    + **Preferred:** 6 months in Acute Care

    Education:

    + **Required:** Bachelor's in counseling

    + **Or Bachelors in:** Health Education, Social Work, Human Services, Psychology or Nursing

    + **Preferred:** Masters in human services

    Credentials:

    + Required - Basic Life Support (Basic Life Support)
