Shift: 9:00am - 5:00pm Shift: Monday - Friday Hours: 9am – 5pm Salary: $ 24.00 Office: Chicago, IL Service Areas: Melrose Park, Chicago, and Will County Offer Based on Years of Experience What You Need to Know Position: Field Supervisor– Non-Clinical POSITION SUMMARY: The field Supervisor is responsible for the effective supervision of Care Partners. This position monitors the activity of Care Partners to ensure that services are being provided to clients in accordance with individualized service plans and state regulations. What does a Field Supervisor do? Complete required quality in-home Compliance visits This necessitates the Field Sup to travel to multiple clients’ homes nearly every day/week. Field Sup will then provide training to their branch’s caregivers as needed. Lead New Hire Caregiver Orientation and training. This is a new caregiver’s first day of employment with us. Completing new client Opening paperwork Meet with new clients. Identify their needs via services. Identify preferred visit cadence (days of week, times of day) Managing casesload up to three hundred 300 clients Field-based role. Local travel is up to 80%. Would need a dependable vehicle, car insurance, valid driver’s license. Qualifications What we look for in a qualified candidate High school diploma or GED required. Strong organizational skills with excellent verbal communication skills. Must be able to work in a variety of environmental conditions. Knowledgable of Microsoft Office Suites Must be sensitive to the needs of the aged and disabled and conscientious about servicing client’s needs. Knowledgeable of all State requirements under the Primary Home Care Program. Ability to handle crisis situations as well as do problem solving intervention. Ability to process time sensitive material in a timely manner.
