    Job Description:

    Facilitates discharge planning and coordination of inpatient care. Actively participates in high utilizer patients care planning and coordination as they present to the hospital. Coordinates post-discharge patient education and engagement utilizing technology tools.

    Qualifications:

    Required:

    Associate Degree in nursing (ADN)

     

    3-5 years clinical experience

    Preferred:

    Bachelor’s Degree in related field

     

    Previous experience in Case Management, Care Management and Discharge Planning
