Title: Case Aide

Program: Fletcher Residence

Salary Range: $18.92/hour

Program Description:

Fletcher Residence is an extended stay, service-enriched single-room occupancy residence providing housing for 55 formerly homeless single adults with chronic mental illness, some of whom are dually-diagnosed with a substance abuse disorder.

Overall Responsibilities:

Under the supervision of the Clinical Coordinator, the Case Aide will work as a key member of an interdisciplinary team, including the Nurse, the Psychiatrist, and Case Managers, in an effort to assist individuals in their recovery and reconnection to community supports.

Essential Duties and Responsibilities:

The essential duties of theCase Aideinclude but are not limited to the following activities:

+ Provide ADL (Activities of Daily Living), and self-management skills training to an assigned caseload.

+ Observe and record client self-administration of medication.

+ Co-lead skills development groups and assist with socialization activities

+ Conduct and document regular room inspections and assess resident ability to maintain a safe, clean environment.

+ Escort residents to community-based appointments and assist with advocacy.

+ Participate in regularly scheduled Clinical Staff and Community Meetings, as well as case planning activities as needed.

+ Document individual and group work in AWARDS software database

+ Provide front desk coverage to relieve staff during lunch and other times as needed

Qualifications:

+ High School Diploma or GED

+ Experience working with adults with mental illness and substance use issues preferred
