    BayCare HomeCare is currently in search of our newest **Home Health Aide** Per Visit who is passionate about providing outstanding customer service to our community. We are looking for an individual seeking a career opportunity with one of the largest employers within the Tampa Bay area.

     

    + **Facility:** BayCare Home Care

    + **Location:** Bradenton, FL

    + **Status:** Per Visit

    + **Shift Hours:** 8:00 AM - 5:00 PM

    + **Weekend Work:** Occasional

    + **On Call:** Occasional

    + **Home visits in assigned service area:** Manatee County, FL

    The Home Health Aide** Per Visit **Responsibilities include:

    + Working under the guidance and supervision of the staff nurse in providing personal care to the patients.

    + Following the patients' plan of care for completion of tasks assigned to a home health aide by the registered nurse or other appropriate skilled professional.

    + Maintaining an open communication process with the patient, representative (if any), caregivers, and family.

    + Demonstrating competency with assigned tasks.

    + Complies with infection prevention and control policies and procedures.

    + Reports changes in the patient condition; and honors patient rights.

    Required Certifications and Licensures:

    + CNA (Certified Nursing Assistant) or HHA certification from accredited 75-hour program

    + Driver's License State of Florida (Florida driver's license is required within 30 days of hire)

    + BLS (Basic Life Support)

    Education:

    + **Preferred:** High School Diploma or equivalent

    Experience:

    + Preferred 1 year Home Care
