    PCC Essential Job Duties:

    + Scheduling and performing Medicare annual wellness visits for assigned providers.

    + Assisting and scheduling patients to maintain up to date preventative services.

    + Work with a multi-disciplinary team to manage care of chronic medical conditions.

     

    Education, Training & Experience

     

    Certified Medical Assistant Details

    Qualifications for CMA Applicants:

    + Graduate from an accredited program for Certified Medical Assistants, Registered. Medical Assistants or Certified Clinical Medical Assistants program.

    + Must have active CMA, RMA or CCMA certificate.

    + BLS certification.

    + Previous CMA experience preferred.

     

    LPN Details

    Qualifications for LPN Applicants:

    + Graduate from a one-year Practical Nursing Program.

    + Currently registered with the Indiana State Board of Nurses Registration and Nursing Education as a Licensed Practical Nurse.

    + BLS certification.

    + Previous LPN experience preferred.
