Job Overview:

RWJBarnabas Health is seeking a highly dedicated RN to work per diem in Case Management for Monmouth Medical Center.

Qualifications:

Required:

+ BSN required.

+ NJ RN License

+ 3 to 5 years acute care RN experience

+ Case Management experience

Preferred:

+ Utilization Review or Discharge Planning Experience

+ Case Management Certification

Scheduling Requirements:

+ 8-4:30 or 9-5

Essential Functions:

+ The Case Manager coordinates quality Healthcare services to meet our patient s needs & assures they are generated in a timely and cost effective manner & helps them and their family make informed Healthcare decisions.
