    Job Description

    Will provide professional social casework in the client’s home in accordance with physician’s orders and agency policies and procedures. Will provide direct social work service to clients in those situations where social/emotional problems exist which are or will be an impediment to the effective treatment of the client’s medical condition or rate of recovery. Will coordinate closely with the primary service provider. Will provide social casework consultation to staff when appropriate.

     

    Will need to earn certification in CPR.
