    What You’ll Do:• 📋 Coordinate Personalized Plans:Develop and implement individualized plans for each client, including Staff Action Plans, Individual Plans of Protective Oversight (IPOP), Personal Expenditure Plans, Money Management Assessments, and Levels of Supervision.• 🤝 Lead Interdisciplinary Teams (IDT):Facilitate team meetings to ensure that clients receive quality active treatment and that their needs are met holistically.• 📅 Monitor Progress:Track and evaluate the progress of clients’ goals and achievements, adjusting plans as necessary.• 📝 Ensure Compliance:Maintain accurate, up-to-date electronic records and ensure all documentation is in compliance with OPWDD (Office for People With Developmental Disabilities) regulations.• 👥 Support Independence:Assist clients in gaining independence with life skills, social interactions, and other personal goals.• 📚 Provide Input for Training Programs:Help develop and implement training programs for staff and clients, monitoring progress and outcomes.

     

    What We’re Looking For:• ✅ Education:Bachelor’s degree in a Human Services field (or related field) required.• ✅ Experience:1+ years of relevant experience in a similar role preferred.• ✅ Skills:Proficiency in person-centered planning, instructional methods, and progress tracking. Strong organizational, communication, and interpersonal skills.• ✅ License:Valid New York State driver’s license is required.• ✅ Knowledge:Understanding of OPWDD regulations and the ability to maintain accurate documentation.

     

    Job Details: 📍 Primary Location:32 Warren Avenue, Tarrytown, NY 🕒 Schedule:Monday–Friday (Full-Time, 40 hrs/week); flexibility required for occasional evenings and weekends. 🚗 Travel Required:Must be willing to travel between IAHD programs in Westchester and the Bronx.
