Chesnut Hill of East Longmeadow , a Skilled Nursing Facility in East Longmeadow, MA, is currently seeking applicants for an experienced MDS Coordinator .

Requirements:

+ Graduate of an accredited school of nursing with current LPN/RN licensure in Massachusetts

+ One year of Long-term care experience

+ Experience with Medicare/Medicaid reimbursement, MDS completion, clinical resources utilization, and/or case management is highly desirable

Responsibilities:

+ Completing accurate assessments, MDS & care plans as assigned

+ Initiating care plans and supporting activities as assigned

+ Creating and distributing monthly care plan calendars in a timely fashion

+ Maintaining and updating care plans and assessments

+ Monitoring and auditing clinical records, ensuring accuracy & timeliness

+ Must be in compliance with MDS / Medicare protocols and policies.

+ Informing DON and ADON of persistent issues related to all non-compliant documentation

+ Additional nursing supervisory tasks as assigned
