    JOB DESCRIPTION

    The Physical Therapist will provide evaluation and treatment for conditions within their scope of practice.

    JOB QUALIFICATIONS

    EDUCATION

    _(For positions requiring education beyond a high school diploma or equivalent, educational qualifications must be from an institution whose accreditation is recognized by the Council for Higher Education and Accreditation.)_

     

    **Required Education** : Bachelor of Science, Masters, or Doctorate degree in Physical Therapy is required.

    EXPERIENCE

    **Preferred/Optional Experience:** Experience in physical therapy is preferred.

    CERTIFICATIONS/LICENSES

    _The following licensure(s), certification(s), registration(s), etc., are required for this position, which must be maintained in good standing:_

     

    **Minimum Required:** Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) within three months of hire. Physical Therapist license awarded by the State of Wisconsin

    **Preferred/Optional:** Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) at time of hire.
