    SIGN ON BONUS-UP TO $1000 **$20.00 Hourly hiring rate**

     

    Several locations throughout Madison County areas, too include Oneida NY; Canastota NY; Chittenango NYDay shifts, evening shifts, weekend shifts!!!

    SUMMARY:TheDirect Support Professionalprovides self-care training and assistance in daily living to individuals with developmental disabilities residing in a group home setting.

     

    ESSENTIAL DUTIES AND RESPONSIBILITIESinclude the following. Other duties may be assigned.

     

    + Provides individualized care as outlined in the individuals Life Plan, or ISP.

    + Assists Individuals in the primary activities of daily living (toileting, feeding, cooking, cleaning) as outlined in individual's ISP to promote independence.

    + Maintains files and produces service notes and documentation on assigned caseload.

    + Documents and submits reports to the program as outlined in the assigned department.

    + Maximized consumer's social skills in order to enable him/her to live in their community by providing activities in a community setting.

    Requirements

    EDUCATION and/or EXPERIENCE:High school diploma or GED; Valid NYS driver's license and 12 months driving history. Six months to one-year related experience and or training preferred.
