    Licensed Practical Nurse - Assertive Community Treatment Program (ACT)

     

    Full-time: Monday-Friday 9-5

     

    Salary: $35-37/hour

     

    Based in Glen Cove, NY

     

    The Charles Evans Center (CEC) is a leading not-for-profit organization located in Nassau County. CEC serves adolescents and adults with addiction and behavioral health needs. We assist clients in achieving their highest level of independence and functioning with the freedom to ASPIRE HIGHER in their hopes, dreams and accomplishments. We are currently seeking a few exceptional professionals who have a passion to create a meaningful impact in the lives of the amazing people we support.

     

    We have a great opportunity for a positive, motivated individual to work in a multi-disciplinary, mobile outpatient OMH program with a specialty in health and self-wellness activities and initiatives for ACT recipients.

    In this position, you will:

    + Work with recipients to develop skills that will support their recovery and attainment of their goals

    + Assist ACT medical staff with health-related activities when needed and appropriate

    + Develop interventions based on OMH's Wellness Self-Management curriculum and other wellness resources

    + Provide mobile community-based services

    We require:

    + Current NYS LPN license

    + Clean, valid NYS driver's license required

    + Must have transportation and be able to travel through Nassau and Suffolk County

    + At least one year of training and/or experience in health and wellness activities for behavioral health recipients, preferred
