    Description:

    Position Summary:

    Accreditation Specialist: Provides support and expertise in the area of external regulatory compliance with standards for inpatient and outpatient elements for Performance.

     

    Supports the development, education, maintenance, consistent application and accountability of Trinity Health Grand Rapids' performance improvement program, in collaboration with leadership staff. Identifies opportunities for performance improvement and conducts or facilitates ways to improve processes and, when applicable, outcomes of care. Provides expertise and education to hospital and medical colleagues on performance improvement strategies, methodology, tools, measurements and documentation of process outcomes. Compiles, analyzes, and evaluates quality data collected as part of an integrated system-wide program of improvement, including effectiveness.

    What the Performance Improvement** **Coordinator will** **need:

    + Bachelor’s degree in a health care field; or an equivalent combination of education and experience.

    + Registered Nurse License strongly preferred.

    + Five years of process improvement experience, including one+ year of experience in leadership or management.

    + Preferred: three years quality management and application performance methodologies.

    + Proven track record in developing, implementing, and managing performance improvement programs at similar sized institutions.

    What the Performance Improvement** **Coordinator will** **do:

    + Responsible to facilitate and support accreditation activities, support survey readiness and survey process for TJC, AAHC

    + Leverages clinical experience to help prepare, plan and respond to citations and improvement efforts.

    + Supports additional initiatives and readiness for certifications as needed.

    + Leads the ministry survey activities, responses and ongoing plans for remediation

    + Communicate to all levels of the organization survey results, complaint surveys and responses.

    + Abstracts and submits clinical data to a variety of local, state and national databases.

    + Facilitates and/or leads process improvement team efforts. Motivates teams to achieve the desired outcomes.

    + Coordinates and facilitates multidisciplinary teams/committees. Monitors outcomes related to real or potential events. Takes initiative to follow through on inquiries, problems or requests.

    + Facilitates ongoing preparedness for accreditation and regulatory agencies. Maintains and uses data and reports from systems to facilitate process improvement. Uses appropriate computer applications in data analysis and to facilitate data-based decisions.

    + Assists in developing quality tools, analysis, data displays, and writing process improvement plans and reports. Develops data presentations in a readily comprehensible way and professional manner.
