Type: Full-Time: 40hrs/week

Salary: $18.31-$18.91/hr DOE

Key Responsibilities:

+ Client Assistance: Assists patients in the clinic setting to provide support and guidance on various resources and benefits.

+ Program Communication: Explain the benefits of the Community Health program to clients/patients and how it can improve their wellbeing.

+ Health Management : Help patients identify socio-economic issues affecting their health and develop management plans.

+ Documentation: Maintain comprehensive electronic client files, documenting encounters, referrals, and outcomes.

+ Service Coordination: Facilitate communication and coordinate services between providers and clients to ensure effective care.

What We’re Looking For:

+ Proficient computer skills, including internet-based applications

+ Ability to thrive in a fast-paced environment while managing multiple priorities

+ Strong organizational skills and accountability for time and activities with remote supervision

+ A passion for serving others with professionalism and clear boundaries

+ Community involvement or membership in the community served

Requirements:

+ High School Diploma or GED

+ Driver's license

Additional Certifications/Trainings upon hire:

+ BLS Certificate

+ Community Health Worker Certificate within the first six months of employment

+ Certified Application's Counselor (CAC) within one year of employment
