    What are we looking for?

     

    We want to add a medical appointment specialist direct support professional (DSP) to our team. The medical appointments specialist DSP coordinates and supports the healthcare needs of individuals with intellectual and developmental disabilities (IDD). This role involves transporting individuals to medical appointments, communicating with healthcare professionals, maintaining accurate and timely documentation, and ensuring follow-up care is completed. The ideal candidate will have strong organizational skills, the ability to problem-solve, and a commitment to enhancing the health and well-being of the people we support. You will collaborate closely with the lead DSP, program director, and state director to ensure that SJCS’s brand voice and messaging are consistently represented across all materials.

    Your overall essential responsibilities will be the following:

    Medical Appointment Coordination

     

    + Transport individuals to and from medical, dental, and therapy appointments in a safe and timely manner.

    + Serve as the primary point of contact during appointments, advocating for the individual and ensuring their needs are addressed.

    + Communicate effectively with healthcare providers to gather and understand diagnosis, treatment plans, and instructions.

     

    Documentation & Communication

     

    + Document all appointment details, including diagnoses, treatment plans, and follow-up recommendations, in Therap or other designated systems within required timeframes.

    + Ensure accurate and comprehensive health-related updates are shared with the individual’s support team, including Residential Team Leaders, Program Managers, and family members as needed.

    + Maintain confidentiality and compliance with HIPAA regulations when handling medical information.

     

    Follow-Up Care

     

    + Monitor and coordinate follow-up appointments, referrals, and prescriptions to ensure continuity of care.

    + Track and manage medical records, lab results, and other health-related documentation.

    + Collaborate with the team to ensure that prescribed treatments, dietary restrictions, and medical equipment needs are implemented and followed.

     

    Individualized Support

     

    + Build rapport with individuals to reduce anxiety related to medical appointments.

    + Demonstrate patience, empathy, and understanding of the unique needs of individuals with IDD.

    + Encourage and support self-advocacy for individuals who can actively participate in their healthcare decisions.

     

    Problem-Solving

     

    + Address and resolve scheduling conflicts, transportation challenges, and unforeseen health-related issues.

    + Proactively identify barriers to accessing care and recommend solutions.

    Other Duties

    + Assist with other DSP responsibilities as needed, including community outings, daily living skills, and personal care.

    + Participate in training and professional development activities to stay informed about best practices in healthcare coordination and IDD support.

    Qualifications

    You will catch our eye if you have the following knowledge, skills, and abilities:

    + High School Diploma or equivalent; additional coursework or certifications in healthcare or IDD services is a plus.

    + At least 1 year of experience working with individuals with IDD or in a healthcare setting.

    + Strong verbal and written communication skills, with the ability to interact effectively with healthcare professionals and support teams.

    + Proficiency in using Therap or similar documentation systems.

    + Valid driver’s license and reliable transportation required.

    + Ability to work independently, prioritize tasks, and demonstrate excellent organizational skills.

    + Knowledge of HIPAA regulations and commitment to maintaining confidentiality.

    Extra credit will be given if you have the following experience, education, and requirements:

    + Familiarity with common medical terminology and understanding of IDD-specific health conditions.

    + Experience in conflict resolution and problem-solving.

    + Empathy, patience, and the ability to build trust and rapport with individuals and their families.

    All SJCS employees must:

    + Obtain a valid health certificate to meet SJCS or current funding authority requirements as required.

    + Meet SJCS and the funding authority requirement of criminal background and record checks.

    Physical and environmental conditions include:

    Physical Demands:

    + Ability to lift, push, or pull up to 50 pounds for assisting individuals or transporting equipment.

    + Capacity to work in various settings, including medical offices, clinics, and individual homes.

    Work Environment:

    + Regular travel to medical appointments and healthcare facilities.

    + Collaborative, team-oriented environment with occasional evening or weekend appointments.

    + Full-time, 40 hours a week with a flexible schedule based on appointment times and location is required.

     

    The hourly rate for this role is $18.00 per hour.
