    Description:

    **Job Title:** Trinity Health at Home Coordinator

    Summary:

    Responsible for facilitating timely and efficient referrals to post-acute homecare and hospice services from the acute care setting. Educates hospital staff and physicians on available services, ensures smooth transitions of care, and supports business development and growth initiatives.

    Key Responsibilities:

    + Coordinate referrals and transitions to homecare services

    + Educate hospital staff on service offerings and eligibility

    + Collaborate with care teams to develop discharge plans

    + Obtain physician orders and insurance verification

    + Serve as a liaison between patients, families, physicians, and agency staff

    + Support business development and new program implementation

    Qualifications:

    + Bachelor’s degree or equivalent experience

    + Clinical licensure preferred

    + 2+ years in acute care, customer service, or healthcare marketing

    + Valid driver’s license and reliable transportation

    + Strong communication and interpersonal skills

    • Maintains positive interpersonal relationships with clients/families, other professionals, co-workers, and the

     

    public. Demonstrates exemplary customer service skills.

    Mental and Physical Requirements:

    • Frequently lift, stands, and walks (35% of time)

    • Occasionally bend, stoop, and twist (10% of time).

    • Occasionally position and lift patients (10% of time).

    • Ability to perform continuously detailed tasks with frequent interruptions and work reprioritizations (90% of

     

    time).

     

    • Must be able to adapt to frequently changing work parameters

    • Must be able to travel to various TH/THHS worksites or various work locations as required
