Responsibilities:

+ Includes individual and group health education counseling, health promotion activities, coordination of community resources and health screenings.

+ Provide services as assigned at various community locations.

+ Play a key role in these activities of the FCN department at community health events, screenings, and educational programs.

+ Support the spiritual, emotional, social and physical health of community members and works closely with FCN leadership to determine department needs.

+ Assist in the Transitional Care Program by introducing program at bedside to potential care recipients.

+ Communicate and provide smooth hand off of transitional care referral to community FCN.

+ Perform Transitional Care visits as per process in efforts to keep patients healthy at home and reduce readmissions to achieve effectiveness throughout the continuum of care.

+ Provide education and intentional care of the spirit to the care recipient as needed at bedside and post discharge.

+ Act as a liaison for community FCN’s who will begin transitional care after discharge for our partner faith communities.

+ Assist FCN Coordinator and FCN Manager of Transitional Care in duties assigned regarding Transitional Care.

+ Perform other duties as assigned.

Minimum Qualifications:

+ Required - Associates - Nursing; Or - Diploma - Nursing

+ Required - RN (Registered Nurse)
