The CNA will be coveringWaterford, Mystic, Lyme, Groton, New London, Stonington and surrounding area. CNA Job responsibilities: * Provides personal care and housekeeping assistance to our clients in their homes; follows the established plan of care under the direction of the client care coordinator. * Provides personal hygiene and grooming care to our clients * Provides meals for clients' nutritional needs * Reports changes in clients' condition to client care coordinator or supervisor * Performs routine housekeeping and home management duties * Records each client's care on care plan and submits care plan in a timely manner * Responds sensitively to the multiple, complex needs of a terminal client which includes providing end of life care. * Assists with routine therapy exercises * Participates in case conferences as requested We take great care of careers With locations around the state, Hartford HealthCare offers exciting opportunities for career development and growth.
