    Description

    Comunilife Inc.is looking for an experiencedCase Manager Supervisorfor itsQueens Emergency Family Shelter, Casa Hope.The Case Manager Supervisor is responsible for the supervision and training and evaluation of the Case Management staff of the Program. Responsible for the coordination of the delivery of social support services to all clients. The Case Manager Supervisor also assists with the intake process of new clients. Responsible for conducting initial and annual psychosocial assessments. Major responsibilities include the administration of the program, monitoring of program services, building relationships and collaborating with other agencies.This position is eligible for Comunilife's generous benefit package including health insurance, dental, vision, dependent care, paid time off, 401(k), tuition assistance program and more.

    Essential Duties

    + Supervise, train and evaluate assigned Case Management staff.

    + Ensure every client is receiving all identified services.

    + Coordinate and supervise efforts with case management and maintenance staff to ensure a minimum occupancy rate is maintained.

    + Coordinate and supervise efforts to place new clients in vacated units within 24hrs.

    + Ensure that the initial and medical assessments are conducted, and that comprehensive service plans are developed that identify mutually agreed upon long and short-term goals.

    + Review and provide supervision in the development of service plans and ensure that reassessments are conducted on a quarterly basis (at minimum) to ensure that services are appropriate for the client’s current needs.

    + Conduct initial and annual psychosocial assessments.

    + Provide oversight and supervision in the delivery of mental health and alcohol/substance abuse services and referrals, including assistance in accessing primary medical care, assistance with adherence to medication regiments, activities for daily living, nutritional services, recreation programs, assistance in employment assessments, training, private sector placement, prevention with positives, and delivery of any other services necessary for clients to increase their capacity for independent living. Ensure that the required documentation is requested and received.

    + Supervise and coordinate efforts to provide assistance in obtaining and maintaining all government benefits and services for which the client is potentially eligible. Ensure that all case managers assist clients who are incapacitated, unable to budget, and/or comply with their fiscal responsibilities by applying for Representative Payee Status, if necessary.

    + Assist and supervise case managers efforts in assisting clients to locate an alternative housing option when a client’s health so requires.

    + Provide regular supervision of the staff performance in the provision of client services.

    + Maintain all documents necessary for quality assurance review by Comunilife, OTDA and/or DHS.

    + Prepare monthly, quarterly, or yearly reports as requested by OTDA and/or DHS.

    + Provide supervision and training to all social services staff. On occasion, accompany social service staff to client sites to assure quality assessments

    + Ensure that all required documentation in the clients’ charts is maintained in compliance with Comunilife and funding standards.

    + Conduct the weekly interdisciplinary team case conferences and provide leadership for these meetings

    + Conduct weekly supervisory conferences with each staff member of the case management teams to review case records and client’s status and progress

    + Contact DHS to arrange intake interviews for new clients and complete intake interview forms for each new client

    + Provide the Program Director with information on the status of new referrals on a weekly basis.

    Qualifications

    Education and Experience Requirement(s)

     

    Minimum: Masters in Social Work with at least four (4) years supervisory experience, overseeing a case management team. Individual, family and group counseling experience required, particularly with mental health and substance abuse and individuals who were formerly homeless. Preference is given to persons with experience in congregate programs, and those who are bi-lingual and bi-cultural in Spanish.
