Intake Coordinator Qualifications:

+ Minimum of 1 year of hospice experience required.

+ Experience in a medical office or healthcare setting is required.

+ Excellent time management skills and detail-oriented tendencies.

+ Must have computer training and Windows-based applications experience required.

+ Must have reliable personal transportation, a valid driver's license, and the ability to safely and personally operate a vehicle.

+ Strong understanding of electronic clinical record utilization.

+ Responsible for maintaining complete confidentiality of patients' records and all other confidential information that may be received during the course of patient care, including financial information and personnel records.

+ Ability to assign a patient to the appropriate team in coordination with the required team leader(s) after admission.

+ Receive and promptly process referrals and/or inquiries for services provided by hospice.

+ Schedule with patient /family, a hospice representative to provide information on hospice care and determine the need for service.

+ Extensive knowledge of hospice care and services.

+ Schedule Option 1: Mon, Tues, Sat & Sun 2:00 PM to 10:30 PM

+ Schedule Option 2: Tues, Wed, Thurs, Fri & Sat 2:00 PM to 10:30 PM
