Responsibilities:
GENERAL PURPOSE
Telemedicine Navigator performs a full range of duties and is responsible for day-to-day operations of telehealth services between the assigned locations and remote sites of care. Establishes and maintains video conferencing connections, makes introductions between patient site and provider site, assists with peripheral equipment, and follows instructions of a clinician to facilitate an effective clinical telemedicine encounter
Coordinates with primary care providers, nurses, social workers, and integrative behavioral health therapists to provide resources and services to address identified social determinants of health resource needs
Responsible for managing telehealth clinical encounters by capturing images, collecting data, and transmitting them electronically for clinical review. Performs diagnostic testing and specimen collection per provider order.
Receives guidance from more experienced staff members and may guide less experienced staff members but requires minimal direct supervision.
ESSENTIAL FUNCTIONS
Navigation:
+ Works in vulnerable communities that often experience barriers to care and high-level trauma.
+ Conducts phone calls for verification/confirmation, insurance and pharmacy verification, and communicates this with the care team.
+ Coordinates with primary care providers, nurses, social workers, and integrative behavioral health therapists to provide resources and services to address identified social determinants of health resource needs. Including but not limited to, identifying and communicating behavioral health needs, transportation, insurance, food, housing and other resource needs.
+ Communicates identified needs to the primary care providers, nurses, social workers, and integrative behavioral health therapists Communicates with Care to facilitate patient care and connect patients to internal and external resources
+ Serves as a navigator within the scope of practice during telehealth encounters to establish and maintain video conferencing connections, makes introductions between patient site and provider site, assists with peripheral equipment, and follows instructions of a clinician to facilitate an effective clinical encounter.
+ Identify resources for clients to overcome barriers to care, such as transportation, housing, and childcare arrangements
+ Maintains documentation of all client encounters and complete reporting requirements according to organization standards.
+ Responsible for signing patients up for MyChart to receive appointment notification reminders, enhance attendance, and decrease no shows.
Technical:
+ As a superuser, configuring, installing, troubleshooting, cleaning, and performing minor routine maintenance for assigned telehealth technology.
+ Responsible for inventory and security of assigned items. Ensures items are ready for use and accounted for by preparing and tracking maintenance work orders, information systems, inventory records, and communicating with the Help Desk to facilitate troubleshooting, repair, and coordination of replacement equipment, as necessary.
+ Navigator maintains regular contact with school health staff, URMC providers, the Facility Telehealth Coordinator and manager and URMC/School IT to work out process issues, equipment needs, problems, data collection, and any other logistical issues.
+ Possesses subject matter expertise in several different areas including procedures, policies, workflows, technology, infection control, and clinical standards.
+ Responsible for training and technical support for other users including new telemedicine navigators & staff, school health staff and community partners, on the telemedicine processes, procedures, and technology.
Clinical/Patient Care:
+ Prepares the clinical environment, which may include preparing medical and telecommunications equipment to be used for clinical examination and treatment.
+ Responsible for managing store and forward telehealth clinical encounters by capturing images, collecting data, and transmitting them electronically for clinical review.
+ Performs diagnostic testing and specimen collection per provider order. Accurately labels, logs and processes specimens according to protocol and follows all Laboratory policies.
+ Acquires vitals and utilizes diagnostic and medical equipment such as but not limited to: Manual Blood Pressure & Automatic Blood pressure machine, pulse oximeter, thermometer, and telemedicine equipment.
+ Provides support by reviewing post visit plan of care with patient and assists with follow up appointment scheduling as appropriate.
Operations:
+ Monitors and maintains schedules of rooms at offsite locations, patients, technologies, clinical providers, and presenters as needed to ensure efficient telehealth operations.
+ Independently manages and coordinates their schedule to effectively and efficiently provide support to patients located at various sites within the Greater Rochester area.
+ Navigator communicates with school health staff, URMC providers, the telemedicine staff, patients and parents/ guardians to facilitate successful telemedicine visit.
+ Adheres to standardized methods, work schedules, work assignments and work safety standards; professional conduct, evaluation and reporting of medical tests, information and results; preventive maintenance equipment procedures and documentation.
+ Identifies and monitors operational changes for improvement to patient flow, utilization and overall telemedicine department functions. Maintains communication with telemedicine staff and direct supervisor to ensure adequate integration and/or synchronization of program operations in effort to maintain alignment with the broader system wide Digital Health telemedicine initiative.
+ Maintains records required for quality control processes and participates in performance improvement activities.
+ Maintains adequacy of clinical equipment and medical supply levels needed for patient care and related to patient safety which may require stocking and cleaning.
+ Conducts inventory of supplies, rotates inventory, monitors expiration dates, reports supply needs.
QUALIFICATIONS
+ Bachelor’s in healthcare related field required.
+ 1-3 years’ experience in community health, human services, and/or patient liaison/advocate roles required.
+ Or equivalent combination of education and experience years’ experience required.
+ Demonstrates excellent communication and problem-solving skills.
+ Comfortable working with patients and families in an urban setting required.
+ Must possess and maintain a valid NYS driver’s license, have a satisfactory driving record and access to reliable transportation required.
+ Must pass URMC required background check required.
+ Strong preference given to candidates with relevant lived experience reflective of our populations, including but not limited to: BIPOC and/or historically minoritized/marginalized individuals, long-term city of Rochester residents, parents/caregivers of children and those familiar with Rochester city schools preferred.
+ Spanish speaking is a plus preferred.
+ Competence in critical thinking, interpersonal relationships, and technical skills required.
+ Ability to work as a team member with excellent communication and customer service skills necessary to effectively contribute to a creative group required.
+ Must be flexible, empathic, and able to engage effectively with diverse populations, including children and teens required.
+ Ability to work in a clinical setting and to communicate effectively with physicians, clinical staff, vendors, and consumers, and facilitate inter-departmental communication in cases where there may be technical overlap required.
+ Demonstrated organizational skills and flexibility to manage multiple tasks and meet deadlines required.
+ Knowledge and understanding of video platforms, tablets, smartphones, their operating systems, processes, and protocols preferred.
+ Ability to work without direction in a networked computer environment required.
+ Formal training and/or experience in trouble shooting and repair of computers and peripherals, including disassembly, cabling, and cable testing preferred.
+ Basic knowledge of computer settings, network settings, firewall issues with video ports, and managing devices for Windows, MacOS, Android, and IOS required.
+ NYS Valid Driver’s License required.
