Responsibilities:
GENERAL PURPOSE
The Social Worker is responsible for providing clinical services to children ages 0-18 who are followed by the Pediatric GI division with a concentration on patients of the Neurogastroenterology and Motility Clinic, as well as their families.
ESSENTIAL FUNCTIONS
+ Provides risk screening/evaluations of Peds Motility patients in the ambulatory setting through chart audit and staff referral. Completes psychosocial assessments of patients identified as requiring social work interventions.
+ Provides social work treatment and supportive counseling to patients and families including; adjustment to diagnosis, progression of disease as well as support of adherence for required medical care.
+ Serves as a resource consultant to patient’s families including; financial counseling for Medicaid/insurance, transportation to appts and meals, comforts during extended clinic visits.
+ Provides pre-admission and post hospitalization planning for patients.
+ Collaborates with members of the health care team around matters related to patient care and enhancing outcomes as well as with other social workers at GCH to ensure continuity of SWK care between settings.
+ Makes referrals to Children’s Health Home or other care management agencies, or social services as indicated.
+ Facilitate communication with schools to support ongoing education and close connection with school nurse involvement
+ Documents social work activity in patient’s medical record in accordance with SWD policy and procedure
+ Identifies and manages all cases of suspected child abuse and neglect in the identified patient population. Provides assistance to patients and families which facilitates compliance with the treatment regimen.
+ Provides coverage to other social work coworkers as needed
+ Other duties as assigned
+ Participates in activities of the Social Work Division including but not limited to designating time spent in the patients medical record, following absence approval process, participating in professional development and completion of other Hospital or Division forms; Hospital and Division Mandatory in-service training programs, and UHS reviews.
+ May serve on a Division Committee
+ May participate in teaching of medical, nursing and social work students/staff
QUALIFICATIONS
+ The Peds GI Social Worker must hold a Master of Social Work degree from an accredited institution required.
+ NY State licensure (LMSW) is required within eighteen months of employment required.
+ Experience in a Pediatric setting with a multidisciplinary team is preferred.
+ Or equivalent combination of education and experience.
+ Knowledge of Psychosocial assessments and family systems preferred.
LICENSES AND CERTIFICATIONS
- LMSW - Licensed Medical Social Worker - State Licensure within 1-1/2 years required or
- If duties will be performed under OMH a minimum of a limited permit (not to exceed 1 year) or NYS Licensure upon hire required
