    Job Summary

    The Enhanced Care Management (EMC) Child Welfare Specialist II is responsible for the design and implementation of strategies to enhance collaboration with county child welfare agencies, ensuring the healthcare needs of L.A. Care’s children and youth in child welfare are met. This position takes a proactive leadership role in advocating for these members by resolving escalated issues, developing, and overseeing care coordination processes, and ensuring compliance with Department of Health Care Services (DHCS) standards.

    Duties

    Primary Point of Contact: Serve as the primary contact for escalated care coordination issues involving L.A. Care child welfare members, designing solutions, and ensuring timely and effective access to services.

     

    Support for ECM Providers and Staff: Provide expert guidance and technical assistance to Enhanced Care Management (ECM) Lead Care Managers and other providers, proactively addressing complex cases and systemic barriers to service access.

     

    Collaboration with Child Welfare Entities: Establish and sustain strategic partnerships with child welfare and county agencies, including the L.A. County Department of Children and Family Services (DCFS). Ensure seamless integration and alignment of ECM services with child welfare initiatives and support the enrollment of eligible children and youth into appropriate programs.

     

    Systemic Issues Resolution: Identify systemic barriers impacting service delivery, lead cross-agency collaboration to address these challenges, and implement solutions that improve outcomes for child welfare members.

     

    Policy Compliance: Design and implement processes to ensure compliance with DHCS requirements and county agreements, supporting accurate documentation, reporting, and the secure exchange of information.

     

    Training and Best Practices: Coordinates training initiatives for ECM providers, focusing on trauma-informed care, county-specific protocols, and best practices to effectively support children and youth in foster care or child welfare systems.

     

    Quality Improvement and Reporting: Collaborate on quality improvement initiatives, represent L.A. Care in quarterly county meetings, and provide actionable insights to refine processes, improve outcomes, and inform future strategies.

     

    Perform other duties as assigned.

    Education Required

    Bachelor's Degree in Social Work or Related Field

     

    In lieu of degree, equivalent education and/or experience may be considered.

    Education Preferred

    Master's Degree in Social Work or Related Field

    Experience

    Required:

    At least 3 years in child welfare services, with experience in care coordination for vulnerable populations.

     

    Skills

    Required:

    Knowledge of child welfare policies, county services, and cross-agency care coordination.

     

    Strong collaboration and relationship management skills with justice and healthcare entities.

     

    Excellent verbal and written communication skills.

     

    Detail oriented, organized and possess time management skills.

     

    Basic computer skills.

     

    Microsoft Office (Word, Excel, Power Point, Access, and Visio).

    Preferred:

    Understanding of Medi-Cal benefits, ECM and the California Advancing and Innovating Medi-Cal (CalAIM), and the LA County Child Welfare System.

     

    Knowledge of L.A. Care core computer system.

     

    Motivational Interviewing skills.

     

    Bilingual in one of LA Care Health Plan’s threshold languages is highly desirable. English, Spanish, Chinese, Armenian, Arabic, Farsi, Khmer, Korean, Russian, Tagalog, Vietnamese

     

    Licenses/Certifications Required

     

    Active & Current Driver's License, with a clean record and Auto Insurance Required

     

    Licenses/Certifications Preferred

     

    Motivational Interviewing Certification
